Unfortunately, over the last several seasons we have seen an increase in unacceptable behaviour on the field during games.  

Please complete the INCIDENT REPORT noted on the left should any issue occur during your games:  racial slurs, confrontations, injuries, etc.

This report is to be completed and submitted to eusa@edisonunitedsoccer.com by the end of the day.  Thank you.
